Although polycystic ovary syndrome (PCOS) is associated with
hyperandrogenism and infertility early in life, it is a harbinger
of a lifelong condition that can lead to serious sequelae such as
endometrial or ovarian cancer, diabetes mellitus, and coronary artery
disease. Insulin resistance and hyperinsulinemia contribute significantly
to its underlying pathophysiology. This paper discusses therapies
that are experimental or are not approved by the US Food and Drug
Administration for the use under discussion. Although the hyperandrogenism
and infertili-ty that PCOS causes are distressing to young women,
its metabolic consequences eventually plague them the most in terms
of morbidity and mortality. Thus, it is crucial to diagnose PCOS early
in its course not only to recognize but also to delay or arrest its

Accutane (Isotretinoin) is approved by the Federal Drug Administration
for the treatment of severe recalcitrant nodular acne that is unresponsive
to conventional therapy, including systemic antibiotics. There are
no other approved indications for the drug and it is not indicated
for milder forms of acne. Accutane is chemically related to both retinoic
acid and vitamin A. Although the exact mechanism of action of Accutane
is unknown, we do know that this drug temporarily suppresses the activity
of sebaceous glands and reduces production of sebum (the fatty secretion
of the sebaceous glands). The reduction of sebum is accompanied by
a marked decrease of the bacteria that is probably responsible for
the inflammation of acne. Although sebum levels and the number of
bacteria slowly return to normal after the treatment is discontinued,

Treating Acne From the Outside: New Combinations Get Ready for the
Medicare Drug Benefit
On December 5, 2000, the Committee on Government Reform, which
I chair, conducted a hearing regarding the potential link between
the acne drug Accutane and depression and suicide. Accutane (isotretinoin)
is a drug approved by the FDA to treat the most serious form of acne
-- a type that is painful, permanently disfiguring, and does not respond
to other acne treatments. However, the FDA has recognized that this
medication has a serious and potentially dangerous side effect. In
some instances, Accutane users become severely depressed and vulnerable
to thoughts of suicide. 1. While Accutane is meant as a treatment
of last resort, it is sometimes prescribed before trying alternatives
with less serious side effects: The FDA approved Accutane for use
for severe nodular cystic acne. The FDA-approved labeling specifically
highlights that this product is not to be considered as a product

1.2 Increased Diagnosis of Acne Physicians use ICDA-9-CM code
706.1 when diagnosing acne vulgaris and acne conglobata; there are
no specific codes available for cystic acne. Table 1 presents the
number of diagnoses for diseases coded as acne NOS or severe acne
NOS alongside information about Accutane new patient starts. In IMS
databases, Accutane is associated 97% of the time with a code of acne
NOS, although some entries include severe acne NOS. As Table 1 shows,
the number of diagnoses for acne and severe acne is increasing, but

Acne vulgaris affects 80% of the population between the ages
of 12 and 25.1 There are various over-the-counter (OTC) and prescription
medica-tions available to treat acne, but only one has shown significant
effects in treating severe acne. The FDA approved Accutane (isotretinoin)
in May 1982, for the treatment of severe recalcitrant nodu-lar acne.2,3
In the last 8 years, there has been a 250% increase in the number
of dispensed prescrip-tions for isotretinoin in the United States.4
