Side Effects:

If this continues or become bothersome or the condition for which this was prescribed does not improve within several days, inform your doctor.

Vision may be temporarily unstable for a few minutes after applying this.

Use caution if driving or performing duties requiring clear vision.

Notify your doctor if you develop a skin rash, itching, eye pain or redness or any other eye discomfort in or around the eyes or a headache while using this medication.

ï Surodex (dexamethosone implant [Oculex]) ï Envision TD (fluocinolone acetonide implant [B&L)] Immunomodulators are potent modifiers of the inflammatory process.
Interferon in various forms is a potent immunomodulator which is gradually finding many applications including the treatment of Hepatitis C, Rheumatoid Arthritis, and Multiple Sclerosis.
A number of the listed medications are cyclo-oxygenase inhibitors that inhibit the enzyme cyclo-oxygenase markedly reducing the synthesis of prostaglandins.
Patanol is an inhibitor of the release of histamine from the mast cells and it is a relatively, selective histamine H1 antagonist that inhibits type 1 immediate hypersensitivity reaction.
Zaditor is a relatively selective, non-competitive histamine antagonist (H1 receptors) and mast cell stabilizer.
Alamast, and Alocril are mast cell stabilizers.
The immunomodulators are valuable for the treatment of giant papillary conjunctivitis and vernal conjunctivitis (Ocufen, Profenal, Feldene, and Lotemax).
Several of the oral medications are valuable for the treatment of cystoid macular edema (Oruvail, Toradal, and Indocin); Indocin ophthalmic solution is also being investigated for the treatment of cystoid macular edema.
The use of Acular and Voltaren and other immunomodulators in combination with ocular steroids is becoming more common with photorefractive keratectomy and their use has helped to reduce complications as well as reducing the mild to moderate pain associated with corneal procedures.
Vexol is a new "soft" steroidal immunomodulator ("soft" medications have more rapid systemic metabolism and excretion via decreased half-lives resulting in reduced side effects).
Vexol is approved for the treatment of anterior uveitis and for post-operative inflammation control.

The amended Manitoba Specified Drug Regulation and Drug Interchangeability Formulary Regulation will be available on the Manitoba Health website http://www.gov.mb.ca/health/mdbif on the effective date of September 15, 2003.
*Abbreviation of Manufacturers' Names **Clozapine: After reviewing the available evidence, the Manitoba Drug Standards and Therapeutics Committee has agreed to designate the new Gen-Clozapine product and the innovator product Clozaril as interchangeable.
Because of the well-known risk of agranulocytosis associated with the use of clozapine, weekly or every two-week hematological testing is necessary prior to dispensing of the next supply of medication.
As a result, distribution of clozapine is regulated through a monitoring system with specific requirements set by Health Canada which involves registration of the patient, physician and pharmacist.
In order to ensure a seamless process and facilitate the work of physicians and pharmacists in switching their patients, Manitoba Health will accommodate the brand name price for a grace period of two (2) months.
The following products will be considered for Pharmacare reimbursement upon an individual prescriber/patient request basis.
Please note that Pharmacare does not cover cyclosporine for the prevention of graft rejection following transplantation.

VEXOL®1 % Ophthalmic Suspension is a sterile, multi-dose topical ophthalmic suspension containing the corticosteroid, rimexolone.
CLINICAL PHARMACOLOGY: Corticosteroids suppress the inflammatory response to a variety of inciting agents of a mechanical, chemical, or immunological nature.
In a controlled 6-week study of steroid responsive subjects, the time to raise intraocular pressure was similar for VEXOL®1 % Ophthalmic Suspension and 0.1 % fluorometholone given four times daily.
After decreasing the dosing frequency to once every two hours while awake during the second week of administration, mean serum concentrations were approximately 100 pg/mL.
INDICATIONS AND USAGE: VEXOL®1% (rimexolone ophthalmic suspension) is indicated for the treatment of postoperative inflammation following ocular surgery and in the treatment of anterior uveitis.
CONTRAINDICATIONS: VEXOL®1% (rimexolone ophthalmic suspension) is contraindicated in epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, varicella, and most other viral diseases of the cornea and conjunctiva; mycobacterial infection of the eye; fungal diseases of the eye; acute purulent untreated infections which, like other diseases caused by microorganisms, may be masked or enhanced by the presence of the steroid; and in those persons with hypersensitivity to any component of the formulation.
Prolonged use may result in ocular hypertension /glaucoma, damage to the optic nerve, defects in visual acuity and visual fields, and posterior subcapsular cataract formation.
Prolonged use may also result in secondary ocular infections due to suppression of host response.
In those diseases causing thinning of the cornea or sclera, perforation has been known to occur with topical steroids.
If this product is used for 10 days or longer, intraocular pressure should be monitored even though it may be difficult in children and uncooperative patients (see WARNINGS).
