Allergic contact reactions from phenylephrine are rare
despite extensive use by ophthalmologists.
Previous reports do not indicate crossreactivity between phenylephrine
and other sympathomimetic drugs in patch testing.
Conclusion: Phenylephrine was the responsible agent for the reactions
in our patients as confirmed by clinical findings and positive patch
tests.
The most frequent side effects of phenylephrine hydrochloride in
eyedrops are blurred vision, burning or stinging, tearing, conjunctival
irritation, liberation of iris pigment into the anterior chamber
and corneal edema.
Phenylephrine hydrochloride is also employed in otic topical preparations
to achieve decongestant effects, and in some rectal and vascular
ointments.

of applying a specific substance to the skin with the intention
of producing a small area of allergic contact dermatitis. allergy,

Genetic polymorphisms in drug receptors, in particular
adrenergic receptors, may contribute to intersubject differences
in pharmacologic response.
Because response to phenylephrine depends upon interaction with
a1B-adrenergic receptors, we tested whether polymorphisms in this
receptor contribute to the variable responses.
Accordingly, we developed a polymerase chain reaction-based method,
generating four exon-spanning fragments, to identify polymorphisms
in the coding sequence of the two exons of the human a1B-adrenergic
receptor.
PCR products of the human a1B-adrenergic receptor gene from human
genomic DNA templets were subjected to electrophoresis on 1% Seakem
GTG-agarose gels.

Method: Patients at the Eye Clinic Srinagarind Hospital
were randomized into two groups.
Pupil diameter, blood pressure and heart rate were measured before
and after eyedrop instillation.
There was no significant difference in mean systolic and diastolic
blood pressure after instillation between the two groups.
However, the mean heart rate after instillation in group 1 was greater
than the value in group 2 with statistically significant difference.
In a prospective randomized double-blind clinical trial there was
a large enough sample size to demonstrate that in darkly pigmented
irides 10% phenylephrine appears to be more effective than 2.5%
phenylephrine in pupillary dilation. phenylephrine, eyedrops.
